A case of severe glutathione synthetase deficiency with novel GSS mutations BJMBR
Xia,H.; Ye,J.; Wang,L.; Zhu,J.; He,Z..
Glutathione synthetase deficiency (GSSD) is a rare inborn error of glutathione metabolism with autosomal recessive inheritance. The severe form of the disease is characterized by acute metabolic acidosis, usually present in the neonatal period with hemolytic anemia and progressive encephalopathy. A case of a male newborn infant who had severe metabolic acidosis with high anion gap, hemolytic anemia, and hyperbilirubinemia is reported. A high level of 5-oxoproline was detected in his urine and a diagnosis of generalized GSSD was made. DNA sequence analysis revealed the infant to be compound heterozygous with two mutations, c.738dupG in exon 8 of GSS gene resulting in p.S247fs and a repetitive sequence in exon 3 of GSS gene. Treatment after diagnosis of GSSD...

A survey of the incidence of neonatal sepsis by group B Streptococcus during a decade in a Brazilian maternity hospital BJID
Vaciloto,Edinéia; Richtmann,Rosana; Costa,Helenilce de Paula Fiod; Kusano,Elisa Junko Ura; Almeida,Maria Fernanda Branco de; Amaro,Eduardo Rahme.
Group B Streptococcus (GBS) is the main etiological agent of neonatal sepsis in developed countries, however there is no detailed information on its incidence in Brazil. We registered the incidence and lethality of GBS infection in a Brazilian private maternity hospital from April 1991 to March 2000. Maternal risk factors contributing to neonatal infections were also scored. The rate of infection was determined by checking for GBS in the blood and liquor of symptomatic neonates within 72 hours of birth. Sepsis and/or early onset meningitis were diagnosed in 43 neonates (32 cases in blood, 1in liquor and 10 in blood and liquor). The overall incidence was 0.39 per thousand neonates and remained quite constant throughout the period, ranging from 0.25-0.63....

Birth weight patterns by gestational age in Brazil Anais da ABC (AABC)
Pedreira,Carlos E.; Pinto,Francisco A.; Pereira,Silvia P.; Costa,Elaine S..
BACKGROUND AND OBJECTIVES: We present an updated birth weight-for-gestational-age portrait, based on nearly 8 million observations of an ethnic-mixed population. It comprises the first comprehensive charts with Brazilian data. This contribution intends to assist clinicians in classifying fetal growth, to provide a reference for investigations of predictors and to show the consequences of small and large patterns for gestational age delivery. Most of the reference data for assessing birth weight for gestational age deal with insufficient sample size, especially at low gestational age. Population-based studies with considerably large sample size refer to data collected more than 15 years ago. METHODS: We accomplished a population-based study on births in all...

C-reactive protein-guided approach may shorten length of antimicrobial treatment of culture-proven late-onset sepsis: an intervention study BJID
Couto,Renato C.; Barbosa,José A. A.; Pedrosa,Tânia M.G.; Biscione,Fernando M..
Late-onset sepsis (LOS) (i.e., sepsis in a neonate after 72 hours of life) is associated with high mortality and significantly prolonged antibiotic exposure and hospital stay in neonates admitted to intensive care units (ICU). In this study, we assessed the reliability of serum C-reactive protein (CRP) as a determinant of antimicrobial treatment duration of LOS. From January 1996 to December 2002, all consecutive infants aged <28 days admitted to a single medical-surgical ICU and diagnosed with primary LOS were enrolled in a prospective, intervention trial with historical controls. Only blood culture-positive LOSs were included. Exclusion criteria were: age >28 days at diagnosis of LOS, development of site-specific infection, and central venous...

Inadequate use of antibiotics and increase in neonatal sepsis caused by resistant bacteria related to health care assistance: a systematic review BJID
Silva,Ana Carolina Bueno; Anchieta,Leni Marcia; Lopes,Marianna Fischer de Paula; Romanelli,Roberta Maia de Castro.
ABSTRACT Background Technologies and life support management have enhanced the survival of preterm infants. The immune system of newborns is immature, which contributes to the occurrence of healthcare-associated infections. The overlap of several conditions with neonatal sepsis and the difficulty of diagnosis and laboratory confirmation during this period result in a tendency to over-treat neonatal sepsis. The use of antimicrobial agents is a risk factor for multidrug-resistant bacterial infections. This work aimed to perform a systematic review of the relationship between inadequate use of antimicrobial agents and increase in neonatal sepsis related to healthcare assistance, due to bacterial resistance. Methods Our population, exposition, comparison,...

Incubação artificial dos ovos e processo de eclosão em Trachemys dorbignyi (Duméril & Bibron) (Reptilia, Testudines, Emydidae) Zoologia
Molina,Flavio de Barros; Gomes,Norma.
Artificial incubation of 558 eggs from 70 clutches of Trachemys dorbignyi (Duméril & Bibron, 1835) were performed at São Paulo Zoo during 1992 and 1993. Hatching occurred when eggs were incubated between 25 and 31.5oC. Incubation time varied from 54 (at 31.5oC) to 120 days (at 25oC), similarly to Trachemys scripla sspp. Hatchling used the caruncle to made small incisions in the egg shell, latter enlarged by movements of the head and forefeet. Hatching usually lasted from one to two days. Newborn's carapace and plastron showed their natural form few hours after the emergence from the egg shell. During the third or fourth week, caruncle usually disappeared, and yolk sac was completely absorbed. Average (x±sd) measures of newborn were 3.55±0.18cm of...

Neonatal Campylobacter coli hemorrhagic enteritis and bacteraemia BJM
Ruiz-Esquide,Fernando; Lafourcade,Mónica; Andrews,Edilia; Fernández,Heriberto.
A case of neonatal campylobacteriosis with hemorrhagic enteritis and bacteraemia due to Campylobacter coli is presented. The mother, from a rural area, had three febrile self-limited diarrheic episodes during pregnancy. The neonate probably acquired the infection during labor. The newborn's serum showed high levels of specific immunoglobulins which could explain the scarce symptoms in this newborn, despite the delay in establishing the gentamicin therapy.

Prevalence of Candida species in umbilical catheters implanted in newborns in Natal, Brazil BJM
Fernandes,Ana Cristina Santos; Sousa Junior,Francisco Canindé de; Oliveira,Solange Maria de; Calich,Lucia; Milan,Eveline Pipolo.
All the newborns with umbilical venous catheters, hospitalized in the neonatal intensive care unit at Januário Cicco Maternity Hospital in Natal, Brazil, between January, 2003 and December, 2004 were studied. The prevalence of Candida species in the tips of intra-venous catheters was assessed, as well as the coexisting exposures that the patients were subjected to. Catheter tips were cultivated in blood agar and when yeast culture occurred, the colony was subcultivated for species identification through morphologic and biochemical assays. From a total of 240 catheters, 41 were positive for yeasts, and 34 were submitted to identification. The following agents were isolated: 13 C. albicans (38%), 10 Candida parapsilosis (29%), 8 C. tropicalis (20%), one C....

Serum levels of vancomycin: is there a prediction using doses in mg/kg/day or m2/day for neonates? BJID
Romanelli,Roberta Maia de Castro; Anchieta,Lêni Márcia; Fernandes,Juliana Chaves Abreu; Lima,Mariana Antunes Faria; Souza,Taís Marina de; Rosado,Viviane; Clemente,Wanessa Trindade; Camargos,Paulo Augusto Moreira.
Abstract Coagulase-negative Staphylococcus has been identified as the main nosocomial agent of neonatal late-onset sepsis. However, based on the pharmacokinetics and erratic distribution of vancomycin, recommended empirical dose is not ideal, due to the inappropriate serum levels that have been measured in neonates. The aim of this study was to evaluate serum levels of vancomycin used in newborns and compare the prediction of adequate serum levels based on doses calculated according to mg/kg/day and m2/day. This is an observational reprospective cohort at a referral neonatal unit, from 2011 to 2013. Newborns treated with vancomycin for the first episode of late-onset sepsis were included. Total dose in mg/kg/day, dose/m2/day, age, weight, body surface and...

Sildenafil prevents the increase of extravascular lung water and pulmonary hypertension after meconium aspiration in newborn piglets BJMBR
Silvera,F.E.; Blasina,M.F.; Vaamonde,L.; Tellechea,S.; Godoy,C.; Zabala,S.; Mañana,G.; Martell,M.; Olivera,W..
Meconium aspiration syndrome causes respiratory failure after birth and in vivo monitoring of pulmonary edema is difficult. The objective of the present study was to assess hemodynamic changes and edema measured by transcardiopulmonary thermodilution in low weight newborn piglets. Additionally, the effect of early administration of sildenafil (2 mg/kg vo, 30 min after meconium aspiration) on this critical parameter was determined in the meconium aspiration syndrome model. Thirty-eight mechanically ventilated anesthetized male piglets (Sus scrofa domestica) aged 12 to 72 h (1660 ± 192 g) received diluted fresh human meconium in the airway in order to evoke pulmonary hypertension (PHT). Extravascular lung water was measured in vivo with a PiCCO monitor and...

The effect of epidural and general anesthesia on newborn rectal temperature at elective cesarean section BJMBR
Yentur,E.A.; Topcu,I.; Ekici,Z.; Ozturk,T.; Keles,G.T.; Civi,M..
Both epidural and general anesthesia can impair thermoregulatory mechanisms during surgery. However, there is lack of information about the effects of different methods of anesthesia on newborn temperature. The purpose of this study was to determine whether there are differences in newborn rectal temperature related to type of anesthesia. Sixty-three pregnant women were randomly assigned to receive general or epidural anesthesia. Maternal core temperature was measured three times with a rectal probe just before anesthesia, at the beginning of surgery and at delivery. In addition, umbilical vein blood was sampled for pH. The rectal temperatures of the babies were recorded immediately after delivery, and Apgar scores were determined 1, 5, and 10 min after...

The Localization of the Asterion According to the Anatomical Landmarks of Posterior Cranial Fossa in Newborns: Clinical Implications International Journal of Morphology
Demir,Mehmet T; Kopuz,Cem; Pirzirenli,Mennan Ece Aydin; Çelik,Fahrettin; Çorumlu,Ufuk.
The purpose of this study was to determine the localization of the asterion according to the anatomical landmarks of posterior cranial fossa and its relation with sinuses for posterolateral surgical approaches in newborns. On 70 head-halves, a needle about 2 mm with diameter was placed on the centre point of asterion (posterolateral fontanel) by inserting into the whole cranial bony tissue by forming an right angle with the bony surface. Various localizations of asterion and its measurements from the internal and external anatomical landmarks were investigated on term neonatal cadavers. The localization of asterion was found as on the sigmoid-transverse sinus junction (STJ) (5., 6., 7., 8. squares) in 40% of cases on right side and in 34%, on left side....

TNF-a and IL-10 levels in tracheobronchial lavage of ventilated preterm infants and subsequent lung function BJMBR
Corso,A.L.; Pitrez,P.M.C.; Machado,D.C.; Stein,R.T.; Jones,M.H..
The role of airway inflammation in ventilated preterm newborns and the risk factors associated with the development of chronic lung disease are not well understood. Our objective was to analyze the association of the airway inflammatory response in ventilated preterm infants by serial measurements of TNF-a and IL-10 in tracheobronchial lavage (TBL) with perinatal factors and lung function measured early in life. A series of TBL samples were collected from ventilated preterm infants (less than 32 weeks of gestational age) and concentrations of TNF-a and IL-10 were measured by ELISA. Pulmonary function tests were performed after discharge by the raised volume rapid compression technique. Twenty-five subjects were recruited and 70 TBL samples were obtained....

Use of a nursery area by cownose rays (Rhinopteridae) in southeastern Brazil Neotropical Ichthyology
Rangel,Bianca S.; Rodrigues,Alexandre; Moreira,Renata G..
ABSTRACT Using non-lethal methods for data collection of individuals and participatory monitoring by fishermen, we provide the first empirical evidence of the use of a nursery area by neonate and young-of-the-year cownose rays Rhinoptera bonasus and R. brasiliensis in southeastern Brazil. Two methods were used to collect data: (1) information provided by fishermen (reports, pictures, and videos) and (2) field sampling by researchers. A total of 746 cownose rays were captured; 113 have been identified as R. bonasus, 15 as R. brasiliensis, and 618 were reported by fishermen and could not be identified to the species. Records of newborns were made only in late spring and summer in 2015, 2016, and 2017, which suggests an annual reproductive cycle, with birth...
